What is reablement care?

Reablement care is designed to help a person get back on their feet as they recover from an illness, injury or surgery. It’s provided upon turning home after being discharged from the hospital. The goal of reablement care is to promote faster recovery, prevent unnecessary hospital readmissions, maximise independent living and eliminate the need for an ongoing care package. It also helps people adjust back to life at home, helping them regain their skills and confidence so that they can once again do things on their own. 

Understanding NHS reablement care

The NHS offers temporary or short-term care to help a person relearn how to do daily activities, like cooking meals and washing, following a hospital stay. The support provided is available as you recover from an illness or an operation so that you can regain your independence. Free care is provided for a maximum of 6 weeks. 

With NHS reablement care, a mix of people from the NHS and social services will support you. The team may include a nurse, occupational therapist,  physiotherapist, social worker, doctor, carer and a speech and language therapist. After assessing your needs and your goals, they’ll set out a plan which will include the contact person, as well as visiting times and dates.
